Hipertansif Hastalarda Gece Kan Basıncındaki Azalmaya Obezitenin Etkisi

Özet
Amaç: Gece kan basıncı düşüşü olmayan kişilerde kardiyovasküler riskin arttığı bilinmektedir. Bu çalışmada hipertansif kişilerde gece kan basıncındaki azalmaya obezitenin etkisi araştırıldı. Gereç ve Yöntem: Beden kitle indeksi (BMI) 30 ve üzerinde olanlar obez kabul edilerek, 35 obez (17 erkek, 18 kadın; yaş ort.: 49,26,3 yıl) ve 36 nonobez (18 erkek, 18 kadın; yaş ort.: 50.16,2 yıl) olmak üzere yeni teşhis edilen veya daha önce teşhis edilmiş fakat düzenli antihipertansif ilaç kullanmayan ve kan basıncı kontrol altında olmayan toplam 71 esansiyel hipertansiyonlu hasta çalışmaya alındı. Tüm hastalarda 24 saatlik ambulatuvar kan basıncı ölçümü yapıldı. Bulgular: Obez olan grupta gece ortalama sistolik kan basıncı yüksek bulundu. Gündüz kan basıncı değerleri ile kıyaslandığında gece kan basıncı düşüşünün obez grupta daha az olduğu tespit edildi. Sistolik kan basıncına göre her iki grubun "dipper" olduğu görülürken, diyastolik ve ortalama kan basınçlarına göre obez grubun "nondipper" olduğu görüldü. Sonuç: Hipertansif hastalar incelendiğinde obezite kardiyovasküler riskin artışına neden olan gece kan basıncındaki düşüşün azalmasına yani nondipping durumuna katkıda bulunmaktadır.
Aim: It is known that cardiovascular risks increase in the individuals without nocturnal blood pressure decrease. In this study, the effect of obesity on the reduction of nocturnal blood pressure decrease in hypertensive subjects was investigated. Materials and Methods: Accepting the individuals with body mass index (BMI) 30 and over as obese, totally 71 patients with essential hypertension, newly or previously diagnosed but not taking antihypertensive drugs, whose blood pressure is out of control, 35 obese (17 male, 18 female; average age: 49.2±6.3 years) and 36 nonobese (18 male, 18 female; average age: 50.1±6.2 years) subjects were included into our criteria. In all patients, ambulatory blood pressure measurements were provided for 24h. Results: In obesity group, nocturnal average systolic blood pressure was found to be high. Compared to the daily blood pressure values, it was detected that the reduction of nocturnal blood pressure was lower in obesity group. Under their diastolic and average blood pressure values, the obesity group was nondipper while both groups were dipper according to their systolic blood pressure values. Conclusion: On investigated hypertensive patients, it was determined that obesity contributes to the decrease of the reduction of nocturnal blood pressure, i.e. nondipping condition, leading to the increase of cardiovascular risks.
