Özet
Bu çalışmanın amacı günümüzde yaygın kullanıma sahip olan titanium kemik içi implantların kemik içine yerleştirildikten sonra kemik implant ara yüzeyinde oluşan yeni kemiğin kalitesine sistemik olarak uygulanan zoledronik asitin etkisini dansitometrik ve histomorfometrik olarak değerlendirmektir. Çalışmamızda 12 adet beyaz genç erişkin yeni Zelanda tavşanı kullanıldı. Randomize olarak seçilen tavşanlar iki gruba ayrıldı. Deney grubuna operasyon sırasında 0,1 mg/kg ZA 15 ml serum fizyolojik ile seyreltilerek İV olarak, Kontrol grubuna ise aynı miktarda serum fizyolojik aynı şekilde uygulandı. Her bir tavşana sağ ve sol olmak üzere 2 şer adet özel yapım titanium implant yerleştirldi. Deney ve kontrol grubundan rasgele seçilen 3 er tavşan 14 günde diğerleri ise 28 günde sacrifiye edildiler. Daha sonra örnekler hazırlanarak densitometrik ve histomorfometrik inceleme yapıldı. Elde edilen veriler ışığında deney grubunda kemik mineral içeriği ve kemik mineral yoğunluğunda 14. gün ve 28. günlerde %33 / %73 KMİ- %24 / %36 KMY istatiksel olarak anlamlı artış belirlendi.(P <0.05) Kontrol grubunda istatiksel olarak anlamlı olmayan %5 artış mevcuttu Non dekalsifiye histomorfometrik incelemede ise yeni kemik oluşumunda 14 gün ve 28. günlerde sırasıyla %23 ve % 27 oranında süreye bağlı olarak istatiksel olarak anlamlı artış saptandı. (p< 0.05) Kemik-implant kontaktı 14 gün ve 28. günlerde sırasıyla %17 ve %18 oranında artış göstermekte olup iki haftalık grupta p=0.223 dört haftalık grupta p=0.037 ölçülmüştür. Çalışmamızda elde edilen bulgulara göre sistemik olarak uygulanan tek doz ZA yeni oluşan kemiğin iyileşme hızını arttırmakta ve kemik kalitesini olumlu bir şekilde etkilemektedir. Gerek daha önce rapor edilmiş olan gerekse sunulan çalışmanın sonuçları her ne kadar olumlu etkilerin varlığını göstersede klinik uygulamaya geçmeden önce osseointegrasyon kalitesine etki edecek optimum dozun ve uygulama şeklinin tespit edilmesi için yeni çalışmalara ihtiyaç vardır.
The aim of this study is to evaluate the histomorphometric and densitometric effect of zoledronic acid administered systemically, to the new developing bone in the bone ? implant interface after insertion of titanium bone implants which are in routine use , nowadays. Twelve white young adult New Zealand rabbits were used in the study. Rabbits were divided to two groups. 0.1 mg/kg Zoledronic ascid diluted in 15 ml serum physiologic was administered IV intra-operatively. Whereas only serum physiologic have been infused intra-operatively to the controls. Two titanium implants were placed to left and right tibia of each rabbit. Randomly selected 3 rabbits of test and control groups were sacrificed at 14th and the others were sacrificed in 28th day. After preparation of samples histomorphometric and densitometric investigations were made. Data revealed a statistically significant increase in bone mineral content and bone mineral density, P<0.05. Non-decalcified histomorphometric examination revealed a time dependent statistically significant increase in newly developing bone 23% and 27%, sequentially, P<0.05. Bone implant contact was also revealed a time dependent increase as 17% and 18%, sequentially compared to the controls and measured as p=0.223 in 2 weeks and p=0.037. Up to our data a single dose of ZA administered systemically increases the rate of new bone development and affects the quality of bone positively.
