COVID-19'da gebelik, doğum ve doğum sonu komplikasyonlar: Karşılaştırmalı retrospektif bir çalışma

Özet
Amaç: Bu çalışmanın amacı doğum için hastaneye yatışı yapılan COVID-19 tanısı alan ve almayan kadınların, gebelik, doğum ve doğum sonuna ait klinik özelliklerinin retrospektif olarak incelenmesidir.
Yöntem: Trakya Üniversitesi Tıp Fakültesinde 17.11.2020- 18.03.2022 tarihleri arasında yapılan çalışmaya doğum için hastane yatışı yapılan COVID-19 tanısı alan 20, COVID-19 tanısı almayan 50 kadının dosyası dahil edilmiştir.
Bulgular: Çalışmaya dahil edilen COVID-19 tanısı almayan kadınların yaş ortalaması 27,60±5,92 iken, COVID-19 tanılı kadınların yaş ortalaması 31,85±5,61’dir. Çalışmaya dahil edilen kadınların çoğunluğu multipar olup COVID-19 tanısı alanların daha düşük gebelik haftasına sahip olduğu tespit edilmiştir. Çalışma sonuçlarına göre, COVID-19 geçiren gebelerde gebelikte folik asit kullanımının daha az ve sigara kullanan kadınlarda daha fazla COVID-19 yaşandığı belirlenmiştir. Ayrıca sezaryen doğum oranının COVID-19 geçiren kadınlarda arttığı ve ten tene temasın daha az gerçekleştiği saptanmıştır. COVID-19 tanısı almış kadınların hastanede kalış süresi, COVID-19 tanısı almamış kadınlara göre daha fazla olup, gruplar arasında anlamlı farklılık saptanmamıştır.
Sonuç: COVID-19 enfeksiyonuna maruz kalan gebe kadınlara bakım için standart önerileri desteklemekte ve bu kadınlar için bakıma erişimin önemini vurgulamaktadır.
Aim: The aim of this study is to retrospectively examine the clinical characteristics of pregnancy, birth, and postpartum women with and without a diagnosis of COVID-19 who were hospitalized for birth. Methods: The files of 20 women diagnosed with COVID-19 and 50 women who were not diagnosed with COVID-19, who were hospitalized for birth, were included in the study conducted at Trakya University Faculty of Medicine between 17.11.2020 and 18.03.2022. Results: While the average age of women not diagnosed with COVID-19 included in the study is 27.60±5.92, the average age of women diagnosed with COVID-19 is 31.85±5.61. Many women included in the study were multiparous and those diagnosed with COVID-19 were found to have a lower gestational age. According to the study results, it was determined that pregnant women who had COVID-19 used less folic acid during pregnancy, and women who smoke had more COVID-19. It has also been found that the rate of cesarean section increases in women with COVID-19 and skin-to-skin contact occurs less frequently. The duration of hospital stays in women diagnosed with COVID-19 was longer than in women without a diagnosis of COVID-19, and no significant difference was detected between the groups. Conclusions: It supports standard recommendations for care of pregnant women exposed to COVID-19 infection and emphasizes the importance of access to care for these women.
