Obez, Diabetik ve Kontrol Gruplarında Serum Leptin Düzeylerinin Kan Basınçlarıyla İlişkisi

Adipozitlerden salgılanan leptin gıda alımı, termogenezis ve beden ağırlığını düzenlemekte rolü olan bir hormondur. Bu çalışmanın amacı serum leptin düzeyleri ile kan basıncı arasında ilişkiyi araştırmaktır. Araştırmaya 71 nondiabetik obez (52 kadın, 19 erkek), 55 tip 2 diabetik hasta (33 kadın, 22 erkek) ve 23 nonobez nondiabetik kontrol grubu (17 kadın, 7 erkek) alındı. Boy ve kilo ölçümüyle beden kitle indeksi, bel ve kalça çevresi ölçümü ile bel kalça oranı hesaplandı. Biseps, triseps, suprailiak ve subskapular bölgesinden deri kıvrım kalınlığı ölçümüyle de standart tablodan vücut yağ yüzdeleri VYY bulundu. 10 dakikalık istirahatten sonra arteriyel kan basınçları ölçüldü. 10-12 saatlik gece açlığından sonra, serum total kolesterol, HDL-kolesterol, LDL-kolesterol, trigilserid ve leptin düzeyleri belirlendi. Obez olgularda arteriyel kan basınçları ile BKI, bel çevresi, VYY arasında pozitif ilişki mevcuttu. Total kolesterol ve LDL-kolesterol düzeyleri ile BKO'nın ilişkili olduğu bulundu. Serum leptin düzeyleri ile arteriyel kan basınçları kuvvetli pozitif ilişkili göstermekteydi. Diabetiklerde, serum leptin düzeyleri obezlerdeki gibi BKI, bel çevresi, VYY ile kuvvetli ilişki göstermesine rağmen yalnızca diastolik kan basıncı ile ilişkiliydi. Sonuç olarak serum leptin düzeyleri ile kan basınçları arasında istatistiksel ilişki bulunmasına rağmen bunun leptinin doğrudan etkisiyle değil obezitedeki artmış düzeyleri ile ilişkili olduğu sonucuna varıldı.
Leptin is secreted by adipocytes and appears to act as a hormone to regulate food intake, thermogenesis and body weight. The aim of this study was to examine the relationship between serum leptin concentrations and blood pressure. We have studied 71 nondiabetic obese (52 female, 19 male), 55 type 2 diabetic patient (33 female, 22 male), and 23 nonobese nondiabetic control subject (16 female, 7 male). The weight, height, waist and hip circumferences were measured and body mass index (BMI), waist to hip ratio (WHR) as values calculated. In all cases biceps, triceps, suprailiac and subscapular skinfold measurements were performed and body fat percentages (BFP) were found comparing standard tables; and blood pressure were measured after resting 10 minutes. After on overnight 12 hours fast; total cholesterol, HDL-cholesterol, LDL-cholesterol, trigliserid and serum leptin levels were determined. In obese cases, there were positive relation between arterial blood pressure and BMI, BFP, and waist circumferences. It was found, total cholesterol and LDL-cholesterol were correlated WHR. Serum leptin levels were strongly positive correlated with arterial blood pressures. In diabetic patients, although serum leptin levels were correlated BMI, BFP, and waist circumferences as obese cases, serum leptin levels were only correlated diastolic blood pressure. In conclusion, although between serum leptin levels and blood pressure were relation statistically, it is concluted that this relation was not the direct effect of leptin, it was related increased levels in obesity.
