Tüberküloz menenjit ekstrapulmoner tüberküloz formları arasında hızlı gidiş, yüksek mortalite ve kalıcı nörolojik sekeller nedeniyle önemini koruyan bir santral sinir sistemi enfeksiyonudur. Bu nedenle erken tanı ve tedavi oldukça önemlidir. Bu çalışmada 1993-1998 yılları arasında kliniğe yatırılarak izlenen 38 tüberküloz menenjit olgusu; öykü, klinik ve laboratuar bulguları yönünden retrospektif olarak değerlendirildi. Olguların %28.9'u 15-25, %44.7'si 25-45 yaş grubunda iken, %26.3'ü 45 yaş üzerindeydi. Kliniğe kabul edildikleri sırada olguların %13'ü evre I, %52. 6'sı evre II ve %34. 2’si evre III tüberküloz menenjit olarak değerlendirilmiştir. 38 olgunun 15'inde (%39.4) şikayetler yatışından 2 hafta veya daha önce başlamış iken, 9'unda (%23.6) 3-7 gün önce başlamış idi. Beyin Omurilik Sıvısı (BOS)'nın Erlich-Ziehl-Nielsen (EZN) yöntemi ile boyanarak direk mikroskopik incelemesinde 13 olguda (%34.2) asidorezistan basil görülürken, sadece 2 olguda Mycobacterium tuberculosis kültürde üretilebilmiştir. Olguların 4'ünde (%10.5) aktif veya geçirilmiş tüberküloz ile ilgili bulgulara rastlandı. 38 olgunun 5'inde (%13.1) kalıcı sekel gelişirken 5'i (%13.1) ölmüştür.
Among extrapulmonary tuberculosis forms, tuberculosis meningitis having a rapid progress, high mortality and permanent neurological sequales, is actually a important central nervous system infection. Therefore, early diagnosis and therapy is very important. In this study, between 1993-1998 years, 38 tuberculosis meningitis patients hospitalized in our clinic ,were evaluated retrospectively about history, clinical and laboratory findings. 28.9% of the patients were between 15-25 years, 44.7% of the patients were between 25 and 45, and 26.3% of them were 45 years old and older. In admission to the clinic ,the patients with tuberculosis meningitis were classified as stage I (13%), stage II (52.6%) and stage III (34.2%) meningitis. Among the 38 patients, in 15 individual (39.4%) the complaints begin 2 weeks or earlier and in 9 of them (23.6%) 3-7 days ago, in the investigation of cerebro spinal fluid, the staining with Erlich-Ziehl Nielsen and directly microscopically findings showed 13 (34.2%) ARB (acid resistant bacilli) positivity, and in only 2 cases Mycobacterium tuberculosis were isolated in cultured media. In 4 cases (10.5%) there were evidence of active or previous tuberculosis. 5 (13.1%) of the 38 patients recovered with permanent sequale and 5(13.1%) of them died.
