Öğe Alternative prosthodontic rehabilitation of traumatic patients with tooth, bone and soft tissue loss: Two clinical reports(2018) Özel, Gülsüm Sayın; İnan, Özgür; Dolanmaz, DoğanAcquired defects typically result from traumatic injuries such as falls, motor vehicle accidents, gunshot injuries or radiation injuries. Those maxillofacial defects resulting from motor vehicle accidents in particular often involve various soft and hard-tissue injuries such as fractures or avulsions of the maxillary or mandibular teeth or fractures of the temporomandibular joints. For a patient suffering such injuries, the retention and stabilization of a prosthesis that contributes to the patient's physical and psychological well-being are often difficult to achieve due to the nature of the moveable tissue beds involved and the resulting lack of support from the remaining teeth and bones. This clinical report describes the treatments of two partially edentulous patients with traumatic injuries that resulted from motor vehicle accidents. The prosthetic rehabilitations were accomplished through the manufacture of (1) a mandibular prosthesis with an extracoronal bar attachment, and (2) a maxillary implant-supported removable prosthesis.Öğe Alveolar Distraction: Analysis of 10 Cases(Mosby-Elsevier, 2002) Uçkan, Sina; Gürsoy Haydar, Seda; Dolanmaz, DoğanObjective. The purpose of this study was to evaluate intraoperative and postoperative complications of alveolar distraction and outcome. Study design. Ten patients with alveolar ridge deficiencies were treated with alveolar distraction osteogenesis by means of intraosseous distractors (Lead system, Stryker Leibenger, Kalamazoo, Mich). The deficiencies were caused by atrophy after periodontal disease or atrophy after tooth extraction (n = 6), benign tumor resection (n = 2), trauma (n = 1), or oligodontia in a case of ectodermal dysplasia (n = 1). The location of the defects was the anterior mandible (7), posterior mandible (1), and anterior maxilla (2). The mean follow-up was 1.8 years, with a range of 10 months to 3 years. Results. The mean alveolar distraction achieved in 10 cases was 8.7 mm (5-15 mm). The intraoperative and postoperative problems encountered were lingual (n = 3) and palatal m = 2) displacement of the distracted segment, fracture of the distracted segment in a very thin alveolar bone (n = 1), and intraoperative bleeding (n = 1). Conclusion. Overall complication rate was 70%. However, most of the complications were minor and eliminated easily. Implant success rate was 85%.Öğe Cerrahi Destekli Hızlı Maksiller Genişletme Sonrası Değişikliklerin Radyolojik Değerlendirilmesi: Derleme Çalışması(Selçuk Üniversitesi, 2022) Alagöz, Elifhan; Ünver, Tuğba; Kurt, Gökmen; Dolanmaz, DoğanKalıtımsal, fonksiyonel veya çevresel etkenler sonucu kafa yüz kemiklerinde ortaya çıkan iskeletsel bir anomali; maksillanın büyümesini olumsuz yönde etkileyerek sagital, vertikal ve transversal yönde maloklüzyonlara neden olabilmektedir. Maksiller transversal darlık, kraniofasiyal alanda görülen iskeletsel anomalilerin en sık olarak gözlendiği problemlerdir. Bu durum hem iskeletsel, hem de dişsel olarak karşımıza çıkabilmektedir. Maksiller darlık vakalarında tedavi alternatifleri yaşa bağlı olarak değişmektedir. İskeletsel maksiller darlık tanısı konmuş olup, gelişimi devam eden bireylerde ortopedik maksiller genişletme yapılmaktadır. Erişkin bireylerde ise maksiller darlıklar, ortopedik kuvvetler yetersiz kaldığı için cerrahi destekli hızlı maksiller genişletme (CDHMG) ile düzeltilebilmektedir. CDHMG prosedürü, plastik cerrahi uygulamasında bir tür kontrollü yumuşak doku genişletmesi olmakla beraber popüler bir kavram olan distraksiyon osteogenezi olarak ifade edildiği bildirilmektedir. Transversal iskelet değişikliklerini değerlendiren çoğu çalışma, dental ölçülere veya konvansiyonel iki boyutlu (2D) radyografilere dayanmaktadır. Ancak 2D radyograflar ile karşılaştırıldığında, üç boyutlu (3D) görüntüleme diş ve iskelet hareketlerinin daha kesin değerlendirilmesini sağlayabilir. Özellikle dental volumetrik tomografi (DVT), invaziv olmama, yüksek erişilebilirlik, yüksek doğruluk ve iyi çözünürlük nedeniyle ağız, diş ve çene-yüz cerrahisinde, ortodontide tanı ve tedavi takibi için benzeri görülmemiş bir teknik haline gelmiştir. 2D go?ru?ntu?nu?n magnifiye olması, distorsiyona ug?raması, anatomik yapıların süperpoze olması gibi bazı kısıtlamalar nedeniyle 2D go?ru?ntu?ler yerine gu?nu?mu?zde 3D go?ru?ntu?ler kullanılmaya bas?lanmıs?tır. Nevertheless, the alveolar deficiency that impedes the insertion of dental implants makes a number of similar reconstructive procedures inevitable. The posterior maxilla is one of the most challenging anatomic locations for the implant placement that requires adjunctive surgical procedures. Diagnosis of this lesion is essential because of its similarity with other odontogenic pathologies. We present three cases of SBC which was firstly diagnosed with plain radiographs that were confirmed with dental computerized tomography. Asymptomatic radiolucent lesions of the mandible should be confirmed with advanced imaging techniques in order to avoid unnecessary surgery.Öğe Distraction Osteogenesis of Basal Mandibular Bone for Reconstruction of the Alveolar Ridge(Churchill Livingstone, 2002) Uçkan, S.; Dolanmaz, Doğan; Kalaycı, Abdullah; Cilasun, U.Alveolar distraction is being used increasingly for alveolar bone reconstruction in patients with severe mandibular defects. When there has been total loss of alveolar bone, distraction of the mandibular basal bone is necessary. Distraction osteogenesis is considerably more challenging in mandibular basal bone than in alveolar bone. The low level of the cut increases the technical difficulty and may result in a poor outcome. We describe three cases in which more than 10 mm of distraction of mandibular basal bone was required. Semirigid distraction devices were used to reconstruct the alveolar structures in each case.Öğe Effect of anatomical location (mandible vs maxilla) of dental implants on the BMP-2, BMP-7, sRANKL and OPG levels in periimplant crevicular fluid during osseointegration. A pilot study(Selçuk Üniversitesi, 2019) Sağlam, Mehmet; Dolanmaz, Doğan; Koçak, Emrah; Gürsoytrak, Burcu; İnan, Özgür; Dündar, Niyazi; Hakkı, Sema S.Background: The aim of this study was to investigate levels of bone morphogenetic protein-2 (BMP-2), BMP-7, soluble receptor activator of nuclear factor-kB ligand (sRANKL) and osteoprotegerin (OPG) in the peri-implant crevicular fluid (PICF) of implants placed in both maxilla and mandible during the osseointegration period. Materials and Methods: Thirty-three patients (17 females and 16 males; mean age 47.03±11.23 years) were included in this study. A total of 33 implants were placed in both of maxilla (Group 1/n=18) and mandible (group 2/n=15). Peri-implant crevicular fluid (PICF) samples, modified plaque index (MPI), gingival index (GI) and probing depth (PD) measurements were obtained at 1 and 3 months after surgery. PICF levels of BMP-2/-7, sRANKL and OPG were analyzed by ELISA. Results: No complications were observed during the healing period. No significant differences were observed in the PICF levels of sRANKL, OPG, BMP-2 and BMP-7 and evaluated clinical parameters between groups at any time point (p>0.05). While PICF volume of group 2 was greater than group 1 at first month, PICF volume of group 1 was greater than group 2 at 3 months (p<0.05). There was a positive correlation between sRANKL levels and PICF volume (p<0.05) and a strong correlation between BMP-2 and BMP-7 (p<0.01). Conclusion: The results of this pilot study didn’t show any significant difference in PICF levels of BMP-2, BMP-7, sRANKL, and OPG in terms of anatomic location of dental implants. Further welldesigned studies should be carried out to evaluate the relationship between bone related biomarkers and anatomic location of dental implants.Öğe Evaluation of Autogenous Grafts Used in Vestibuloplasty(Cambridge Med Publ, 2003) Metin, Muhammet; Dolanmaz, Doğan; Alkan, ArzuVestibuloplasty is indicated when prosthesis stability is poor due to mandibular or maxillary atrophy. The use of autogenous grafts to provide sufficient vestibular depth is widely accepted. In this retrospective study, the outcomes of vestibuloplasty performed on 41 patients using various types of autogenous grafts (full-thickness skin, dermal, reversed dermal, meshed skin and palatal mucosal) are presented. The best results were obtained with palatal mucosal grafts, but graft size is limited. Meshed skin grafts were shown to be the best alternative.Öğe Experimental Evaluation of Acute Molding of the Regenerate in Mandibular Distraction Osteogenesis in a Rabbit Model(Mosby-Elsevier, 2010) Çandırlı, Celal; Dolanmaz, Doğan; Etöz, Osman A.; Avunduk, Mustafa CihatObjectives. The aim of this study was to investigate the possible effect of acute regenerate molding (ARM) on bone healing in mandibular distraction osteogenesis (DO). Study design. Nine white New Zealand rabbits underwent unilateral mandibular lengthening via DO. Three groups were created: The first underwent 15 degrees ARM, the second group underwent 30 ARM after the distraction period, and the third group served as control without molding. After 1 month of consolidation, all of the animals were killed and histomorphometric evaluation was performed. Results. New bone formation was uneventful in all of the groups. In the ARM groups, the amounts of osteoblasts and newly formed bone areas were higher in compressed areas than in stretched ones (P < .05). The regenerates in the control group and stretched regenerate areas of the ARM groups were similar regarding new bone formation (P > .05). Conclusion. Acute regenerate molding could be safely applied immediately after the distraction period to correct postdistraction deformities without compromising bone healing.Öğe Experimental Evaluation of the Effects of Zoledronic Acid on Regenerate Bone Formation and Osteoporosis in Mandibular Distraction Osteogenesis(W B Saunders Co-elsevier Inc, 2006) Pampu, A. Alper; Dolanmaz, Doğan; Tüz, Hakan H.; Karabacakoğlu, AydınPurpose: The aim of this study was to evaluate the effects of systemically administered zoledronic acid (ZA) on the bone mineral density (BMD) and bone mineral content (BMC) at mandibular distraction sites in rabbits. Materials and Methods: Eighteen New Zealand white rabbits were randomly divided into 2 groups. Bone lengthening was performed in the left portion of the mandible through distraction osteogenesis at a rate of 0.5 mm every 12 hours for 5 days. While the experimental group rabbits were administered intravenous 0.1 mg/kg ZA, control group rabbits were given only saline infusion during operation. All animals were sacrificed at the end of the consolidation period of 28 days. The mandibles of all animals were removed and both the anterior and posterior pin regions of the regenerate and regenerate region were evaluated by dual energy x-ray absorptiometry (DEXA). BMD and BMC data were statistically analyzed. Results: Except for 1 rabbit from the experimental group that had an infection at the external pin region, all animals showed complete clinical healing. When the values in the group receiving ZA were compared with those of the control group, it was observed that the BMD values of the anterior pin region of regenerate, regenerate region, and posterior pin region increased by 23%, 20%, and 31%, respectively; and BMC values increased by 22%, 24%, and 32%, respectively. When data of these regions were compared, both BMD and BMC were found statistically different in all regions (P <.05). Conclusion: Results of this study showed that ZA had positive effects on new bone formation at and around the distraction gaps of the lengthened rabbit mandibles by distraction osteogenesis.Öğe Garre Osteomyeliti (Proliferatif Periostitis): Vaka Raporu(2003) Dolanmaz, Doğan; Metin, MuratGarre osteomiyeliti yada proliferatif periostitisle birlikte kronik osteomiyelit, periosteumun düşük dereceli enfeksiyonlara karşı gösterdiği, tabakalar halinde yeni kemik oluşumuyla karakter ize nadir görülen bir patolojidir. Sıklıkla mandibular molar dişlerden kaynaklanan periapikal bir enfeksiyon sonucu oluşur. Bunun yanı sıra diş çekimi sonrası ya da yan sürmüş molar dişlere bağlı gelişen bir enfeksiyon sonucunda da meydana gelebilir. Tipik olarak mandibula posteriorda ve genellikle unilateraldir. Hastalar karakteristik olarak normal görünümlü mukoza ve cildin örttüğü kemik sertliğinde asemptomatik bir şişliğe sahiptir. Tedavisinde önemli olan tanının konulması ve etkenin ortadan kaldırılmasıdır. Bunun ardından genellikle mandibula ilave cerrahiye gerek kalmaksızın aşamalı olarak remodele olur. Bu çalışmada 14 yaşındaki bir erkek hastada var olan Garre osteomiyelitinin teşhis ve tedavisi sunulmaktadır.Öğe Histomorphometric Examination of Long-term Changes in Temporomandibular Joints After Mandibular Lengthening by Distraction Osteogenesis in Rabbits(Elsevier Science Inc, 2012) Mihmanlı, Ahmet; Dolanmaz, Doğan; Tüz, Hakan; Pampu, Alper; Dönmez, Hasan HüseyinObjective. The purpose of this study was to evaluate long-term histomorphometric changes in temporomandibular joints (TMJs) of rabbits after mandibular distraction osteogenesis (DO). Study Design. Twenty-six rabbits were used in this study. Two of them served as control subjects, and the remaining 24 underwent DO procedures in their left mandibular bodies. After a latency period of 5 days, 5 mm lengthening was performed at a rate of 1 mm/d. The rabbits in the experimental group were randomly divided into 4 subgroups and killed after 1, 2, 4, and 6 months. TMJs from both sides were harvested and prepared with hematoxylin and eosin stain for histomorphometric examination under an optical microscope. Results. Compared with control subjects and nondistracted sides, fibrous articular, proliferative, and hypertrophic areas were significantly increased (P < .05) in the first 2 months in distracted sides. The changes were insignificant in the fourth and sixth postoperative months. Conclusion. Unilateral mandibular distraction of 5 mm was found to be well tolerated and no degenerative changes were observed histologically in the rabbit TMJs in the long-term period.Öğe İki farklı laterognati vakasında sabit ortodontik tedaviyle birlikte tek çene cerrahi yaklaşımı: Vaka raporu(Selçuk Üniversitesi, 2019) Soğancı, Ahmet Ertan; Esen, Alparslan; Lale, Bekir; Dolanmaz, DoğanAmaç: Bu vaka raporunun amacı; iki farklı laterognati vakasının, sabit ortodontik tedavi ve sadece alt çeneyi içeren cerrahi yaklaşım sonrası meydana gelen değişimlerini sunmaktır. Olgu 1: 20 yaş 2 aylık kadın hasta alt çenesindeki asimetri sebebiyle kliniğimize başvurmuştur. Yapılan ağız dışı fasiyal cephe değerlendirmesinde alt çene ucunun yaklaşık 4 mm kadar sola deviye olduğu laterognati görüntüsü mevcuttu. Hastada underjet ve sol bölgede posterior çapraz kapanış mevcuttu. Olgu 2: 20 yaş 3 aylık kadın hasta alt çenesindeki asimetri sebebiyle kliniğimize başvurmuştur. Yapılan ağız dışı fasiyal cephe değerlendirmesinde alt çene ucunun 6 mm sağa deviye olduğu laterognati görüntüsü mevcuttu. Sağ bölgede posterior çapraz kapanış mevcuttu. İlk olgu için 15 aylık, ikinci olguda ise 17 aylık sabit ortodontik tedaviden sonra bilateral sagittal split osteotomisi, uygulanmıştır. Sonuç: Tedavi edilen iki farklı laterognati olgusunda; sabit ortodontik tedaviyle birlikte sadece alt çeneye uygulanan cerrahi yaklaşım; ciddi asimetriye sahip hastaların fasiyal ve dental laterognati anomalisinin düzeltilmesini gerçekleştirerek, tatmin edici sonuçlar alınmasını sağlamıştır.Öğe Impacted tooth in mandibular fracture line: Treatment with closed reduction(W B SAUNDERS CO, 2004) Baykul, Timuçin; Erdem, Erdal; Dolanmaz, Doğan; Alkan, AlperPurpose: Most mandibular fractures occur because of blunt trauma. In some cases, the mandible can be weakened by the presence of an unerupted or impacted tooth. The fate of this tooth in the fracture line and its effects on bone healing remain an important outcome event. Materials and Methods: In this study, we analyzed the records of 117 patients with a total of 121 fractures having an unerupted tooth remaining in the fracture line who were treated and followed by the Dental Faculty of Ankara University between 1986 and 1996. All were open fractures, and none of the patients had a history of an acute inflammatory episode related to the unerupted tooth before the injury. Intermaxillary fixation was used in all cases. Results: Three of the patients subsequently required open reduction with internal fixation. The unerupted tooth was not extracted in any patient. The fixation was maintained for an average of 45 days and followed up for 3 months. No complications were encountered. Conclusion: According to these results, we suggest that asymptomatic impacted teeth in the fracture line of uncomplicated mandible fractures do not cause delayed healing when treated with closed reduction. (C) 2004 American Association of Oral and Maxillofacial Surgeons.Öğe Kemik içi vida destekli molar distalizasyonu(2004) Gelgör, İbrahim Erhan; Büyükyılmaz, Tamer; Karaman, Ali İhya; Dolanmaz, DoğanHasta kooperasyonunu en aza indirmeyi amaçlayan ağıziçi molar distalizasyon apareylerinin gelişmesiyle birlikte son yıllarda kemik içine veya üzerine yerleştirilen implant/onplant sistemlerinden destek alınarak gerçekleştirilen diş hareketleri konusunda çalışmalar artmaktadır. Bu çalışmanın amaçları: 1) Üst çenede molar distalizasyonu için premaksiller bölgeye yerleştirilen bir kemikiçi vida yeterli ankraj sağlayabilir mi sorusuna cevap aramak, 2) Bir kemikiçi vida ve iki premolar dişten destek alınarak gerçekleştirilen üst molar distalizasyonu sonrası sagital ve vertikal boyutlar ile yumuşak dokularda meydan gelen değişikliklerle beraber kesici ve azı dişlerindeki değişikliklerin incelenmesidir. Çalışma dişsel sınıf II iskeletsel sınıf I malokluzyona sahip, çekimsiz bir tedavi planı yapılmış, yaşları 11 yaş 3 ay ile 15 yaş 6 ay arasında değişen 9 kız 2 erkek toplam 11 birey üzerinde yürütülmüştür. Üst çenede palatinal bölgede incisiv kanalın yaklaşık 0.5 mm. gerisinden, yine orta hatta maksiller sütüre denk gelmeyecek şekilde bir tarafa daha yakın olarak yerleştirilen vidadan destek alınarak birinci azıların ve varsa ikinci azıların distalizasyonu süper sınıf I azı ilişkisi elde edilene kadar sürdürülmüştür. Distalizasyon süresi ortalama 4,6 aydır. Uygulama sonucu görülen değişiklikler, distalizasyon öncesi ve sonrası alınan sefalometrik film ve üst çene alçı modelleri üzerinde incelenmiştir. Yapılan ölçümlerde üst 1. büyükazı dişlerinin sefalometrik olarak ortalama 10 ' lik bir eğilme ile 3,6 mm. distalize olduğu saptanmıştır. Model ölçümlerine göre üst 1.büyükazı dişlerinde ortalama 5 mm. distalizasyon sağlanırken aynı zamanda distopalatinal yönde bir rotasyon meydana gelmiştir. Distalizasyon sonrasında santral kesici dişler ortalama 0,7 mm. protruzyona uğramıştır. Overjet ve overbite'ta herhangi bir artış olmazken mandibular düzlem açısında bir değişiklik gözlenmemiştir. Tüm verilerin ışığında: hastaların ihtiyacına göre yeterli miktarda distalizasyon sağlanırken ankrajın korunduğu saptanmıştır.Öğe Management of Alveolar Clefts Using Dento-osseous Transport Distraction Osteogenesis(E H ANGLE EDUCATION RESEARCH FOUNDATION, INC, 2003) Dolanmaz, Doğan; Karaman, Ali İhya; Durmuş, Ercan; Malkoç, SıddıkThis article evaluates the usage of distraction osteogenesis (DO) in the treatment of cleft alveoli. The procedure was carried out on eight alveolar clefts of five patients between the ages of 17 and 25 years. Three patients had bilateral alveolar clefts (BAC) and two patients had unilateral alveolar clefts (UAC). DO was carried out bilateral to the palatal segments for the BAC patients and unilateral to the lesser segment for the UAC patients. A custom-made tooth-borne distractor was used. The average amount of distraction was eight mm (range, 5-11.5 mm). The average amount of distal movement of the anchorage teeth was 0.8 mm (range, 0-2 mm). The average amount of inclination changes of the transport segments and anchorage teeth was 7.6degrees (range, 2-17.5degrees) and 3.3degrees (range, 0-9degrees), respectively. Two important problems were observed attributable to the method. First, the transport segment was docked in a more superior position at the end of distraction process. This undesirable movement Also changed the inclination of the teeth in the transport segment and increased tooth tipping. Removing the device in the second week of the consolidation period and retracting the segment to its ideal position orthodontically solved these problems. Second, the bony defect on the nasal side of the alveolar cleft could not be completely closed. This method for repairing small or large alveolar clefts is a simple, cost-effective, and useful treatment option. However, repairing the alveolar cleft without grafts seems to be impossible when using a tooth-borne device.Öğe Maxillary Anterior Segmental Advancement by Using Distraction Osteogenesis: a Case Report(E H ANGLE EDUCATION RESEARCH FOUNDATION, INC, 2003) Dolanmaz, Doğan; Karaman, Ali İhya; Özyesil, Atilla GökhanAfter the first clinical application of distraction osteogenesis (DO) to correct mandibular deformity was reported in 1992, various applications such as maxillary or midface advancement, temporomandibular joint reconstruction, alveolar augmentation, and mandibular widening have been described in the oral and maxillofacial region. Block et al and Altuna et al first examined anterior segmental DO experimentally in the maxilla and reported successful results. After these studies, DO has been used clinically for the total advancement of the maxilla or midface. But no clinical application of DO for maxillary anterior segmental advancement was found by a review of the literature in English. In this article, we present a case with a skeletal Class III abnormality resulting from a maxillary deficiency, which was treated by using anterior segmental DO.Öğe Odontogenic Myxoma Containing Osteocement-Like Spheroid Bodies: Report of A Case with An Unusual Histopathological Feature(MUNKSGAARD INT PUBL LTD, 2001) Oygür, Tülin; Dolanmaz, Doğan; Tokman, Benay; Bayraktar, SerhatThe odontogenic myxoma is a rare, benign, but locally invasive tumour of the jaws. Radiographically, it is a bone destroying lesion and has ill-defined borders. Histological characteristics are spindle and stellate-shaped tumour cells and a distinct myxomatous stroma. Bony islands that represent residual trabeculae are found scattered throughout the lesion. In the orthodontic treatment of an ankylosed tooth, different treatment modalities have been put into practice including both orthodontic and orthodontic-surgical approaches. For favorable results, gingival margin esthetics must be considered as much as leveling the ankylosed tooth in the arch. Distraction osteogenesis accompanied by orthodontic mechanics is a sensible way of achieving this goal. However, devices used in the distraction protocol are high in price and bulky in shape, causing functional and esthetic problems for the patient. This report describes treatment of an infrapositioned ankylosed incisor with continuous distraction forces produced by conventional orthodontic mechanics. In conclusion, the ankylosed tooth was leveled in the upper arch successfully with a harmonic gingival margin.
