Öğe Axis I and Axis II diagnoses in patients with chronic idiopathic urticaria(PERGAMON-ELSEVIER SCIENCE LTD, 2008) Uguz, Faruk; Engin, Burhan; Yimaz, ErtanObjective: This study presents the current prevalence of Axis I and Axis II psychiatric diagnoses and factors associated with the existence of Axis I psychiatric disorders in patients with chronic idiopathic urticaria (CIU). Method: The study sample was composed of 89 patients with CIU and 64 control subjects. Axis I and Axis II psychiatric disorders were ascertained by means of the Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition and the Structured Clinical Interview for DSM, Revised Third Edition Personality Disorders, respectively. Results: Of patients with CIU, 44 (49.4%) had at least one Axis I diagnosis, and 40 (44.9%) had at least one personality disorder. The most common Axis I disorder was obsessive-compulsive disorder (25.8%), and the most common Axis II disorder was obsessive-compulsive (30.3%) personality disorder in patients with CIU. Obsessive-compulsive disorder, major depression, obsessive-compulsive and avoidant personality disorders were more prevalent in patient group compared to control group. Obsessive-compulsive and avoidant personality disorders were related to the existence of Axis I disorders in patients with CIU. Conclusion: Psychiatric morbidity seems to be a frequent healthy problem in patients with CIU. (c) 2008 Elsevier Inc. All rights reserved.Öğe Evaluation of application of multiple needle pricks on the pathergy reaction(WILEY, 2008) Ozdemir, Mustafa; Bodur, Said; Engin, Burhan; Baysal, IbrahimBackground The pathergy reaction (PR) to a needle prick is a well-known feature of Behcet's disease (BD). Although PR is accepted as a major criterion in this disease, there is a lack of a standardized method to measure PR. The aim of this study was to evaluate the effect on the positive rate of PR of the application of numerous (one to six) needle pricks. Methods Six needle pricks were performed on 105 patients with BD. The results of the test were noted from the proximal to the distal region for each area of each patient. The positive rates of all combinations (one, two, three, four, five, and six needle pricks) were calculated as percentages by a computer-based program (SPSS version 13 for Windows), and the mean percentage rates were obtained. Results There was an increase in the percentage of positive PR rates in each combination relative to the former combination. When one needle prick was performed, the mean percentage positive PR rate was 13%. The positive PR rates were 19%, 24%, 28%, 30%, and 33% for two, three, four, five and six needle pricks, respectively. The greatest increase in the mean percentage of positive PR rates was detected for the application of the two needle prick combination. Conclusion There is no consensus about the number of needle pricks required to detect PR, and multiple needle pricks have been used in many studies. Two components are responsible for skin ageing: Intrinsic (genetic) and extrinsic (photo ageing). Intrinsic ageing is related to genetic background of individual. On the other hand, extrinsic ageing which can be prevented and delayed, is mostly related to smoking, alcohol intake, increased sun exposure, poor nutrition and hormones. Since life expectancy has increased, interests to anti-ageing have been increased. There are many topical preparations that are now being used or under investigation for anti-ageing. Even though topical medications such as tretinoin have been demonstrated in the literature to reduce the signs of ageing, patients often seek over the counter anti-ageing products. Some of the most popular ingredients used in these products are vitamins, minerals and botanical extracts. In this article, ingredients of the anti-ageing products are reviewed. Bu çalışmada OST ve hücresel immünite belirleyicilerinin ilişkili olup olmadıklarını araştırdık. GEREÇ-YÖNTEM: Bu amaçla 68 hasta ve 37 kontrol grubuna OST ve tüberkülin deri testi (TDT) uygulandı ve T-lenfosit alt grupları ölçüldü. BULGULAR: OST kontrol grubuna göre ürtikerli hastalarda anlamlı olarak pozitif bulundu. TDT yanıtı 32 hastada pozitifti. T-lenfosit alt grupları CD4: %41.33.4, CD8: %346.5, CD4/CD8: %1.2, CD3: %668.8 şeklindeydi. SONUÇ: Sonuç olarak kronik ürtiker hastalarında OST’nin, TDT yanıtı ve T-lenfosit alt grupları ile ilişkili olmadığını belirledik.Öğe Neutrophilic figurate erythema(WILEY, 2008) Ozdemir, Mustafa; Engin, Burhan; Toy, Hatice; Demirkesen, CuyanWe herein report a 39-year-old woman presented with annular plaque with central clearing on the back, extremities, and breasts. The lesions had raised, firm erythematous borders included a trailing scale. On histologic examination, a perivascular inflammatory cell infiltrate composed of neutrophils and mononuclear inflammatory cells in the upper dermis was shown. We consider our case to represent idiopathic figurate erythema with a dominant infiltration of neutrophils.Öğe Purified Protein Derivative Skin Test in Patients with Psoriasis Vulgaris: A Case-Control Study(Ortadogu Ad Pres & Publ Co, 2010) Özdemir, Mustafa; Engin, BurhanObjective: The diameter of purified protein derivative skin test (PPD) induration in evaluation of positive reaction varies according to patient's immune status, living area and BCG vaccination. A needle prick trauma and PPD extract may lead to Koebner phenomenon after 72 hours and these applications may be the cause of false positive or increase of diameter of PPD reaction in psoriatic patients. The aim of the study was to evaluate the results of PPD skin test in psoriatic patients and to compare with control subjects. Material and Methods: The test was performed on 117 consecutive patients with psoriasis vulgaris and 102 immunocompetant control patients. The Mantoux method was used for the test and skin reaction was measured at 72 hours. A positive skin reaction was considered > 10 mm. Results: A positive PPD test was detected in 49 psoriatic patients (41.8%) and in 31 of the control subjects (30.3%). Differences in positive PPD reactions (p=0.16) and the diameter of PPD indurations (p=0.36) between two groups were not statistically significant. The diameter of PPD indurations in psoriatic patients was not associated with sex, duration of the disease or psoriasis area and severity index score (p> 0.05). Conclusion: Our results show that psoriasis has no effect on the PPD skin test. We think that this test is appropriate to recognize latent tuberculosis infection in patients with psoriasis until more sensitive diagnostic tests become available.Öğe Quality of life in patients with chronic idiopathic urticaria: the impact of Axis I and Axis II psychiatric disorders(ELSEVIER SCIENCE INC, 2008) Uguz, Faruk; Engin, Burhan; Yilmaz, ErtanObjective: Chronic idiopathic urticaria (CIU) associated with lower quality of life (QoL) levels. Psychiatric disturbances are frequently present in patients with CIU. This study examined the impact of Axis I and Axis II psychiatric disorders oil the QoL of patients with CIU. Method: The study sample consisted of 100 subjects including CIU patients with only Axis I psychiatric diagnoses (n=25), CIU patients with only Axis II diagnoses (n=25), CIU patients with both Axis I and Axis II diagnoses (n=25), CIU patients without any Axis I and Axis II diagnosis (n=25), and healthy control subjects with respect to urticaria and psychiatric disorders (n=25). The Structured Clinical Interview for Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) (SCID-I) and the Structured Clinical Interview for DSM, Revised Third Edition Personality Disorders (SCID-11) was used to determine Axis I and Axis II psychiatric disorders, respectively. QoL levels were assessed by means of the World Health Organization QoL Assessment-Brief (WHOWOL-BREF). Results: There were significant differences among the groups with respect to all WHOQOL-BREF subscale scores. The QoL levels were similar among CIU patients with only Axis I psychiatric diagnoses, CIU patients with only Axis II diagnoses and CIU patients with both Axis I and Axis It diagnoses, and between CIU patients without any Axis I and Axis II diagnosis, and healthy control Subjects. When compared with those of CIU patients without any Axis I and Axis II diagnosis, CIU patients without concurrent psychiatric diagnoses had significantly lower scores in most domains of the QoL scale. Conclusions: Axis I and Axis II psychiatric disorders seem to be considerable factors influencing the QoL in CIU patients. (C) 2008 Elsevier Inc. Ail rights reserved.Öğe A Randomized Comparison of Acitretin-narrow-band TL-01 Phototherapy and Acitretin-Psoralen plus Ultraviolet A for Psoriasis(ACTA DERMATO-VENEREOLOGICA, 2008) Oezemir, Mustafa; Engin, Burhan; Baysal, Ibrahim; Mevlitoglu, InciThe combination of retinoids with phototherapy enhances the efficacy of phototherapy and reduces the cumulative ultraviolet dose and duration of the therapy needed to treat chronic plaque psoriasis. Although TL-01 phototherapy has been used widely, there are few data about the effectiveness of the combination of acitretin with TL-01 in treatment of the disease. The aim of this study was to compare acitretin-narrow-band TL-01 phototherapy with acitretin-psoralen plus ultraviolet A (acitretin-PUVA) in psoriasis. We studied 60 patients with moderate to severe plaque psoriasis who were randomly allocated to three times weekly treatment acitretin-narrow-band TL-01 or acitretin-PUVA. The efficacy of treatments was assessed using the Psoriasis Area and Severity Index by a blinded observer. Clearance of psoriasis was achieved in 56.6% of patients treated with acitretin-narrow-band TL-01 and in 63.3% of those treated with acitretin-PUVA. All of these patients remained clear of psoriasis 3 months after finishing the treatments. Mucocutaneous side-effects, such as dry lips and mouth, were the most common complaints in both groups. In conclusion, acitretin-narrow-band TL-01 is an effective and well-tolerated treatment for moderate to severe plaque psoriasis, with a therapeutic effect equal to that of acitretin-PUVA.Öğe Remitting Seronegative Symmetrical Synovitis With Pitting Edema in a Patient With Kaposi Sarcoma(LIPPINCOTT WILLIAMS & WILKINS, 2008) Oezdemir, Mustafa; Engin, Burhan; Tunc, Recep; Mevlitoglu, Inci[Abstract not Available]Öğe Treatment of Angiokeratoma of Fordyce with Long-Pulse Neodymium-Doped Yttrium Aluminium Garnet Laser(WILEY-BLACKWELL PUBLISHING, INC, 2009) Oezdemir, Mustafa; Baysal, Ibrahim; Engin, Burhan; Oezdemir, SunaAngiokeratomas are typically asymptomatic, blue-to-red papules with a scaly surface located on the scrotum, shaft of penis, labia majora, inner thigh, or lower abdomen. The treatment of angiokeratomas may be necessary if they bleed and lead to patient anxiety. To determine the safety and effectiveness of long-pulse 1,064 neodymium-doped yttrium aluminium garnet (Nd:YAG) laser for the treatment of angiokeratomas of Fordyce. Ten consecutive patients with angiokeratoma of Fordyce were treated with long-pulse Nd:YAG laser in two to six sessions. The three authors independently assessed improvement of the lesion based on digital photographs taken before the treatment and 2 months after the end of the treatment. Significant (> 75%, < 100%) and moderate (> 50%, < 75%) improvement was seen in six and two patients, respectively. Complete improvement was achieved in one patient. Transient swelling, purpura, bleeding, and some pain in the treated area were noted in all patients as short-term side effects. There were no permanent side effects. The long-pulse Nd:YAG laser is a highly effective and safe treatment for angiokeratoma of Fordyce. The authors have indicated no significant interest with commercial supporters.Öğe Treatment of chronic urticaria with narrowband ultraviolet B phototherapy: a randomized controlled trial(ACTA DERMATO-VENEREOLOGICA, 2008) Engin, Burhan; Oezdemir, Mustafa; Balevi, Ali; Mevlitoglu, InciData regarding narrowband ultraviolet B (NB-UVB) phototherapy in patients with chronic urticaria is limited. The aim of this open, controlled study was to determine whether NB-UVB is effective in treating urticaria in combination with antihistamin. A total of 81 patients with chronic urticaria were recruited, 48 of whom were randomized into the NB-UVB plus antihistamine group. The control group (n = 33) received only antihistamine. Patients were assessed using the urticaria activity score and a visual analogue score (VAS). The 2 groups were evaluated at the same time-points: at treatment sessions 10 and 20 and at follow-up 3 months post-treatment. The reduction in urticaria activity score and VAS was statistically significant (p<0.05 for both groups). When comparing the groups, the mean urticaria activity score was significantly lower in the NB-UVB group at session 10 (22.6 vs. 27.3) and session 20 (17.4 vs. 20.7). Statistically significant differences were also noted in VAS between the 2 groups (p<0.01) at 3 months post-treatment. We conclude that NB-UVB may be an effective complementary treatment for patients with chronic urticaria.Öğe Treatment of Faun-Tail Naevus with Intense Pulsed Light(MARY ANN LIEBERT INC, 2010) Ozdemir, Mustafa; Balevi, Ali; Engin, Burhan; Guney, Figen; Tol, HueseyinBackground: Faun tail is a rare cutaneous marker of spinal dysraphism. This neurological abnormality may lead to difficulties such as severe pain and burning sensations in treatment of hypertrichosis of faun tail with laser or laser-like devices. Objective: We evaluated outcomes of an intense pulsed light source in two patients with faun tail. Methods: The Lumina intense pulsed light system [650-nm handpiece (550-1100 nm)] was used for the treatment. Magnetic resonance imaging and neurological examination were done. Results: Tethered cord syndrome was detected as a neurological abnormality. The patients were treated with an energy fluence of 18-26 J/cm(2), pulse sequencing of 3 to 4, and a delay time of 20-35 ms. Local anesthesia was applied in one patient during treatment for severe pain sensation. A mean of 85% hair reduction was achieved. Conclusion: A good cosmetic result with intense pulsed light treatment was achieved in the patients with faun tail.
