Multipl Sklerozda Yaşam Kalitesinin Depresyon ve Anksiyete ile İlişkisi

Özet
Amaç: Multipl sklerozda (MS) yaşam kalitesinin depresyon, anksiyete, fiziksel özürlülük ve hastalık süresi ile ilişkisinin araştırılması amaçlandı, yöntem: Araştırmaya 62 MS'li hasta ve 34 sağlıklı-gönüllü kişi kontrol grubu olarak alınmıştır. Hasta ve kontrol grubuna Beck Depresyon Envanteri (BDE), Hamilton Depresyon Derecelendirme Ölçeği (HAM-D), Hamilton Anksiyete Değerlendirme Ölçeği (HAM-A), Yasam Kalitesi Ölçeği (WHO-QOL-Brief) uygulanmıştır. Ayrıca, hastalara Kısa Yeti Yitimi (KYY) ve Genişletilmiş Özürlülük Durum Skalası (EDSS) uygulanmıştır. Sonuçlar: Hastaların %67.7'sikadın, %32.3'3'üerkek olup, ortalama hastalık süresi 4.5pm5.1 yıl, ortalama EDSSpuanı 1.5pm1.4 olarak bulunmuştur. MS hastalarında depresyon ve anksiyete puanları kontrollerden anlamlı derecede yüksek, yaşam kalitesi puanları düşük bulunmuştur. Depresyon puanları ile yaşam kalitesinin tüm alanları, anksiyete puanları ile ise fiziksel sağlık, psikolojik sağlık ve sosyal ilişkiler alanı arasında negatif yönde ilişki bulunmuştur. Hastalık süresi ve yeti yitimi ile yaşam kalitesinin arasında negatif ilişki bulunurken, doktorun değerlendirdiği fiziksel özürlülük skoru ile yaşam kalitesi ilişkili bulunmamıştır. Tartışma: Depresyon ve anksiyete MS hastalarında yasam kalitesini olumsuz yönde etkilemektedir. MS hastalarında doktorun değerlendirdiği fiziksel özürlülük skoru ile yaşam kalitesinin ilişkili bulunmamasına karşın, hastanın bildirimine dayanan yeti yitimi puanı ile ilişkili bulunması ilginçtir.
Objective: In this study was to investigate the relationships between depression, anxiety, illness duration and physical disability and quality of life in multiple sclerosis (MS). Method: Sixty-two patients with MS and thirty-four healthy-voluntary individuals were included in the study. Patients and control group have been evaluated with Beck Depression Inventory (BDE), Hamilton Depression Rating Scale (HDKS), Hamilton Anxiety Rating Scale (HAM-A), Quality of Life Index (WHO-QOL-Brief). Patients were additionally evaluated with Brief Disability Questionnaire (KYY), Expanded Disability Status Scale (EDSS). Results: Of the 62 MS patients 67.7% were women and 32.3% were men. The mean duration of illness was 4.5pm5.1 years, EDSS was 1.5pm1.4. Depression and anxiety scores of MS patients were found to be significantly higher than controls, however their scores of quality of life were low. A negative correlation was foundbetween depression scores and all quality of life domains. Anxiety scores of the patients were also negatively correlated with physical and psychological well-being and social relationship domains. Illness duration and disability were negatively correlated with quality of life. Physical disability scores detected by the physician was not found to be correlative with quality of life. Discussion: The quality of life in MS is negatively influenced by anxiety and depression. In the patients there is no correlation between physical disability scores evaluated by the physician and the quality of life. However it is surprising to find a correlation between disability scores defined by the patients and the quality of life.
