Özet
Sağlık kurumlarına ve sağlık hizmet sunucularına yapılan geri ödeme mekanizmaları, sağlık hizmet sunumunun kalitesi ve verimliliği açısından çok önemli bir etkiye sahiptir. Ödeme yöntemlerinden her biri sağlık hizmetlerinde kaliteyi, üretim miktarını, sağlık hizmetlerinin etkinliğini aynı zamanda da hem hizmet sunan hem de hizmet alan tarafı olumlu ve olumsuz etkileyebilen farklı özelliklere sahiptir. Bu nedenle ödeme sistemlerinden tek bir yöntemi temel alarak kullanmak yerine birden fazla ödeme yöntemini bir arada kullanmak daha iyi bir yol olarak görülmektedir. Yine ülkemizde de birden fazla geri ödeme yönteminin birlikte kullanıldığı söylenebilmektedir. Ödeyiciler tarafından sağlık hizmeti kurumlarına ve sağlık profesyonellerine yapılan ödeme yöntemleri ise şunlardır; global ve harcama kalemlerine göre bütçe yöntemi, kişi başına ödeme, hizmet başına ödeme, prim ödemesi, vaka başına ödeme (TİG), maaş yöntemi ve gün başına ödemelerdir (Beylik vd., 2015: 40). Bu çalışma sağlık hizmetlerinde kullanılmakta olan geri ödeme yöntemlerinin neler olduğunun hem sağlık kurumu açısından hem de bireysel hizmet sunucuları açısından incelenmesi amacıyla gerçekleştirilmiş bir çalışmadır. Çalışmada her bir yöntemin hem güçlü hem de zayıf yönleri ele alınmaktadır.
Reimbursement mechanisms to health institutions and health care providers have a significant impact on the quality and efficiency of health care delivery. Each of the payment methods has different characteristics that can affect the quality and quantity of production and the effectiveness of health services both positively and negatively. Therefore, instead of using a single method based on payment systems, it is seen as a better way to use multiple payment methods together. Again, it can be said that more than one payment method is used together in our country. Payment methods made by the payers to health care institutions and health professionals are as follows; According to the global and expenditure items, the budget method is pay per person, pay per service, premium payment, pay per case (TIG), salary method and pay per day (Beylik vd., 2015: 40). This study was conducted to examine the reimbursement methods used in health services both in terms of health institution and service providers. Both strengths and weaknesses of each method were discussed in the study.
